Overview

Afrezza + basal insulin is a combination therapy used for the treatment of diabetes mellitus, particularly type 1 and type 2 diabetes. **Afrezza** is a rapid-acting, inhaled formulation of regular human insulin used to control blood glucose levels at mealtimes. It is administered via oral inhalation. **Basal insulin** refers to long-acting insulins (such as insulin degludec, glargine, detemir, or others), delivered by subcutaneous injection, providing steady background insulin coverage throughout the day and night. The combination of Afrezza for prandial (mealtime and correction) insulin and a basal insulin for background coverage aims to mimic the body's natural insulin profile, optimizing glycemic control and reducing HbA1c. Afrezza has a faster onset and a shorter duration of action compared to subcutaneous rapid-acting insulin analogs, which may lower the risk of late postprandial hypoglycemia but is associated with risks such as bronchospasm in patients with lung disease and the need for pulmonary monitoring. The combination's efficacy and safety have been evaluated in multiple clinical studies and is approved for managing type 1 diabetes (in combination with a basal insulin) and type 2 diabetes (with or without oral agents).
