AG + FOLFIRINOX is a sequential chemotherapy regimen being investigated as a neoadjuvant and adjuvant treatment for patients with resectable pancreatic cancer. The regimen consists of two distinct phases: the AG regimen, which combines nab-paclitaxel (albumin-bound paclitaxel) and gemcitabine, followed by the FOLFIRINOX regimen, which includes oxaliplatin, irinotecan, leucovorin (folinic acid), and 5-fluorouracil (5-FU). This sequential approach is designed to utilize multiple cytotoxic mechanisms—including microtubule stabilization, DNA synthesis inhibition, DNA cross-linking, and topoisomerase I inhibition—to maximize therapeutic efficacy and improve surgical outcomes in pancreatic adenocarcinoma. The regimen is currently being evaluated in a Phase II clinical trial sponsored by the Tianjin Medical University Cancer Institute and Hospital.
