Aganepag isopropyl is a small molecule drug under investigation for the treatment of glaucoma and ocular hypertension. It acts as a selective agonist of the prostanoid EP2 receptor, leading to increased levels of intracellular cyclic adenosine monophosphate (cAMP) in ocular tissues such as the ciliary body and trabecular meshwork. This mechanism promotes aqueous humor outflow through both trabecular and uveoscleral pathways, thereby lowering intraocular pressure (IOP). The drug was initially developed by Allergan UC and has been studied in clinical trials comparing its efficacy to bimatoprost ophthalmic solution in patients with glaucoma or ocular hypertension[1][2][3][5].
