AGG-523 + tolbutamide is an investigational combination consisting of **AGG-523**, a selective oral inhibitor of the metalloproteinases ADAMTS-4 and ADAMTS-5, and **tolbutamide**, a first-generation sulfonylurea used as an oral hypoglycemic agent for type 2 diabetes mellitus. AGG-523 aims to attenuate elevated aggrecanase activity and protect cartilage in osteoarthritis by inhibiting the proteolytic enzymes responsible for extracellular matrix degradation. Tolbutamide acts by stimulating pancreatic β-cell insulin secretion via sulfonylurea receptor binding, thereby lowering blood glucose levels. The combination has not reached the market and is cited only in investigational contexts, with AGG-523 having undergone early-phase osteoarthritis trials. No evidence was found that this combination is approved or widely used clinically[1][2][3][4][6][8].
