AGI-134 is a fully synthetic alpha-Gal glycolipid designed as an intratumoral cancer immunotherapy for solid tumors. Upon intratumoral injection, AGI-134 labels cancer cell membranes with the carbohydrate antigen alpha-Gal (aGal), which is not naturally expressed in humans but is recognized by pre-existing anti-alpha-Gal antibodies present in human serum due to exposure to gut bacteria. The binding of these antibodies to the treated tumor cells activates complement-dependent and antibody-dependent cellular cytotoxicity (ADCC), leading to direct tumor cell destruction and release of tumor-associated antigens. This process creates a pro-inflammatory microenvironment and induces a systemic anti-tumor immune response—effectively turning the patient’s own tumor into an autologous vaccine. Preclinical studies have shown that AGI-134 can cause regression of primary tumors and prevent growth of secondary metastases; it also demonstrates synergy when combined with immune checkpoint inhibitors such as anti-PD-1 antibodies[1][2][4][5][6]. The drug was originally developed by Agalimmune Ltd., later acquired by BioLineRx.
