Aglatimagene besadenovec is an investigational, replication-defective adenoviral vector-based gene therapy designed to deliver the herpes simplex virus thymidine kinase (HSV-tk) gene directly into tumor cells. Once inside the cell, HSV-tk converts administered prodrugs such as valacyclovir or ganciclovir into toxic metabolites, selectively killing transduced cancer cells and inducing a local immune response against the tumor. This multimodal approach combines direct cytotoxicity with immunostimulation and is being developed primarily for solid tumors including prostate cancer, pancreatic ductal adenocarcinoma, non-small cell lung cancer, glioblastoma multiforme, and other high-grade gliomas. The drug is administered via intratumoral injection and has shown positive results in Phase 3 trials for intermediate-to-high-risk localized prostate cancer when combined with standard of care radiation and valacyclovir[1][2][4][5][6][7][8][9].
