AGP100 is a first-in-class, small molecule phosphodiesterase 2 (PDE2) inhibitor being developed by Agiana Pharmaceuticals for the treatment of catecholaminergic polymorphic ventricular tachycardia (CPVT). CPVT is a rare, inherited arrhythmic disorder characterized by life-threatening ventricular arrhythmias triggered by physical or emotional stress, often due to mutations in the ryanodine receptor 2 (RyR2) or calsequestrin 2 (CASQ2). By inhibiting PDE2, AGP100 aims to modulate cyclic nucleotide (cAMP and cGMP) signaling pathways within cardiac myocytes to stabilize calcium handling and prevent the triggered activity that leads to arrhythmias. The drug has successfully completed Phase 1 safety and tolerability studies in healthy adults and elderly populations and is currently advancing into Phase 2a clinical trials to evaluate its efficacy in CPVT patients who exhibit residual ectopy despite standard-of-care treatment.
