AHCT intensification + daratumumab + lenalidomide is a combination treatment regimen evaluated in the MASTER-2 trial (NCT05231629) for patients with newly diagnosed multiple myeloma (NDMM) who are minimal residual disease (MRD) positive. The regimen utilizes autologous hematopoietic cell transplantation (AHCT) as an intensification strategy, followed by consolidation and maintenance with daratumumab and lenalidomide. Daratumumab is a human IgG1κ monoclonal antibody that targets CD38, a protein highly expressed on myeloma cells, inducing cell death through antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), and complement-dependent cytotoxicity (CDC). Lenalidomide is an immunomodulatory drug (IMiD) that binds to cereblon, part of the E3 ubiquitin ligase complex, leading to the degradation of transcription factors Ikaros and Aiolos, which results in anti-proliferative and pro-apoptotic effects in myeloma cells while enhancing T-cell and NK-cell activity.
