AHCT intensification + teclistamab + daratumumab is a multi-phase combination treatment regimen evaluated in the MASTER-2 phase 2 clinical trial (NCT05231629) for patients with newly diagnosed multiple myeloma (NDMM). The regimen is specifically utilized for patients who remain minimal residual disease (MRD) positive after induction therapy with daratumumab, bortezomib, lenalidomide, and dexamethasone (Dara-VRd). It consists of autologous hematopoietic cell transplantation (AHCT) using melphalan conditioning as an intensification step, followed by consolidation and maintenance phases using a combination of teclistamab (a BCMA x CD3 bispecific T-cell engager) and daratumumab (a CD38-targeting monoclonal antibody). This MRD-guided approach aims to achieve deep, sustained responses by intensifying therapy in patients with persistent disease after initial induction.
