Aidfarel is a biosimilar of aflibercept, a recombinant fusion protein that functions as a soluble decoy receptor for vascular endothelial growth factor (VEGF) and placental growth factor (PlGF). Developed by Intas Pharmaceuticals, Aidfarel is designed to treat various retinal vascular disorders by inhibiting pathological angiogenesis and reducing vascular permeability. The molecule consists of the extracellular domains of human VEGF receptors 1 and 2 fused to the Fc portion of human IgG1. By binding to VEGF-A, VEGF-B, and PlGF with higher affinity than their natural receptors, Aidfarel prevents these ligands from activating VEGFR-1 and VEGFR-2 on endothelial cells. This blockade effectively suppresses the proliferation of abnormal blood vessels and the associated leakage in conditions such as neovascular (wet) age-related macular degeneration (nAMD), diabetic macular edema (DME), and macular edema following retinal vein occlusion (RVO).
