AININ-20 (also known as AININ20) is a monoclonal antibody fragment (specifically a single-chain variable fragment, scFv) that selectively targets the pathogenic cis-conformation of phosphorylated tau protein at threonine 231 (cis-pT231 tau). Developed by the Royan Institute and clinically evaluated by the Islamic Azad University Medical Branch of Tehran, AININ-20 is designed to prevent the abnormal aggregation of tau proteins and the formation of neurofibrillary tangles, which are key drivers of neurodegeneration in Alzheimer's disease and traumatic brain injury (TBI). By neutralizing this toxic tau conformer, AININ-20 aims to halt the spread of pathology through neural circuits and preserve cognitive function. It is currently being evaluated in a Phase 1 clinical trial for mild to moderate Alzheimer's disease and has also been investigated in preclinical models of traumatic brain injury.
