AIR-CF2 is an experimental dry powder inhalation formulation of the aminoglycoside antibiotic tobramycin, developed by Alkermes in collaboration with Eli Lilly. Utilizing Alkermes' proprietary AIR pulmonary delivery technology, the drug was engineered as large, porous, low-density particles designed to be delivered efficiently to the lower respiratory tract using a simple, breath-actuated inhaler. The primary indication for AIR-CF2 was the management of chronic *Pseudomonas aeruginosa* infections in patients with cystic fibrosis (CF). Tobramycin exerts its antibacterial effect by binding to the bacterial 30S ribosomal subunit, which inhibits protein synthesis and leads to cell death. Despite reaching Phase 2 clinical trials, the development of AIR-CF2 was discontinued after Eli Lilly returned the program rights to Alkermes in the early 2000s.
