AK130 is a first-in-class tri-specific antibody designed to simultaneously inhibit the Programmed cell death protein 1 (PD-1), Cytotoxic T-lymphocyte protein 4 (CTLA-4), and Vascular endothelial growth factor (VEGF) pathways. Developed by Akeso, the molecule utilizes a rational, avidity-based design to enable tumor-selective immune checkpoint blockade. Specifically, the antibody's affinity for CTLA-4 is markedly enhanced only in the presence of PD-1, a mechanism that preferentially enables CTLA-4 inhibition on dual-expressing activated T cells within the tumor microenvironment. This approach is intended to improve the therapeutic index by reducing the systemic immune-related toxicities typically associated with conventional CTLA-4 blockade. Concurrently, AK130 neutralizes VEGF to inhibit tumor angiogenesis and modulate the immunosuppressive microenvironment. Preclinical data presented at AACR 2026 demonstrated superior tumor growth inhibition compared to PD-1 or CTLA-4 monotherapies and robust anti-angiogenic activity, supporting its further clinical development for solid tumors.
