AK135 is a novel small molecule drug candidate and a derivative of 1,4-naphthoquinone (menadione). It has demonstrated the ability to restore electron flow in defective mitochondrial respiratory chains more effectively than menadione, particularly under conditions of hypoxia or inhibition by agents such as rotenone. Mechanistically, AK135 acts as an inhibitor of interleukin-1 receptor accessory protein (IL1RAP), representing a new approach for modulating inflammatory pathways. The primary clinical indication under investigation is chemotherapy-induced peripheral neuropathy (CIPN) in patients with malignant tumors. Early-phase clinical trials are ongoing to assess its safety, tolerability, pharmacokinetics, and preliminary efficacy[1][2][4][5][8].
