AKP-11 is a first-in-class small molecule drug developed by Akaal Pharma. It acts as a selective agonist of the sphingosine-1-phosphate receptor 1 (S1P1), modulating immune cell trafficking and reducing inflammation. Unlike earlier S1P1 modulators such as fingolimod, AKP-11 downregulates S1P1 independently of sphingosine kinase activity and demonstrates immunomodulatory effects with a more favorable safety profile, including milder and reversible lymphopenia and reduced cardiovascular side effects in preclinical models. The drug is being developed for both topical and oral administration for autoimmune diseases (including rheumatoid arthritis, atopic dermatitis, plaque psoriasis), inflammatory skin disorders, neuropathic pain, acne vulgaris, alopecia areata, multiple sclerosis, ulcerative colitis, osteoarthritis, gout, actinic keratosis, Crohn's disease and diabetic retinopathy[1][3][4][5][6].
