AL-78898A is a synthetic cyclic peptide and a derivative of compstatin that acts as a potent inhibitor of complement factor C3 activation. By binding to complement component C3, it prevents its cleavage into active fragments (C3a and C3b), thereby blocking the downstream activation of the complement cascade. This mechanism is relevant in diseases where overactivation of the complement system contributes to pathology, such as age-related macular degeneration (AMD). AL-78898A was developed for intravitreal administration in ophthalmic indications, particularly for geographic atrophy and wet AMD. The drug reached phase 2 clinical trials but development was discontinued for these indications[1][2][5][6][7].
