AL04 is a recombinant fusion protein therapeutic designed for the treatment of Alzheimer's disease. The molecule is a tripartite construct consisting of human Cystatin C, human serum albumin (HSA), and a proprietary blood-brain barrier (BBB) penetrating peptide. Cystatin C serves as the therapeutic moiety, acting as a natural inhibitor of cysteine proteases and a modulator of amyloid-beta aggregation and tau phosphorylation. The HSA component is integrated to extend the circulatory half-life of the protein, while the BBB-penetrating peptide facilitates the delivery of the large fusion protein across the blood-brain barrier into the central nervous system. Preclinical studies have demonstrated that AL04 can effectively reduce amyloid-beta plaque deposition and hyperphosphorylated tau levels, potentially through the activation of protein phosphatase 2A (PP2A) and the enhancement of protein degradation pathways.
