Al18F-NOTA-FAP-2286 is a radiopharmaceutical PET imaging tracer targeting fibroblast activation protein-alpha (FAPα). It consists of the FAP-targeting peptide ligand FAP-2286 conjugated to the chelator NOTA and labeled with the radioisotope fluorine-18 via an aluminum fluoride ([Al18F]2+) complex. FAP is highly expressed in the cancer-associated fibroblasts (CAFs) of various solid tumors but has low expression in normal tissues, making it a high-contrast target for molecular imaging. This tracer was developed to overcome limitations of 68Ga-labeled analogs, such as the short half-life of Gallium-68, by utilizing the longer half-life and superior imaging characteristics of Fluorine-18. It is particularly being explored for imaging PSMA-negative or PSMA-resistant prostate cancer and other FAP-expressing malignancies.
