**Al18F-NOTA-FAPI-74** is a fluorine-18 radiolabeled fibroblast activation protein inhibitor PET imaging tracer. It comprises the small-molecule FAPI-74 ligand conjugated to the NOTA chelator and labeled through an aluminium-[18F]fluoride complex. The agent binds **fibroblast activation protein** on cancer-associated fibroblasts in the tumor microenvironment, enabling PET/CT visualization of FAP-expressing tumors and metastases. It originated from the Heidelberg FAPI program and is being clinically developed and manufactured in the United States by SOFIE as an oncology diagnostic and potential companion diagnostic, including Phase 3 studies in pancreatic ductal adenocarcinoma and gastroesophageal malignancies. Fluorine-18 labeling provides a 110-minute physical half-life and supports centralized, larger-scale radiopharmaceutical production relative to gallium-68 FAPI tracers.
