Al18F-NOTA-pentixafor is a radiopharmaceutical imaging agent designed for positron emission tomography (PET) imaging of tissues expressing the C-X-C chemokine receptor type 4 (CXCR4). The tracer consists of the high-affinity cyclic pentapeptide CXCR4 ligand, pentixafor (also known as CPCR4-2), which is conjugated to the chelator NOTA and labeled with the radioisotope fluorine-18 using an aluminum-fluoride ([Al18F]2+) complexing method. CXCR4 is a G-protein-coupled receptor that plays a critical role in cell migration and is significantly overexpressed in various conditions, including hematological malignancies, certain solid tumors, and aldosterone-producing adenomas (APA) in patients with primary aldosteronism. By binding specifically to CXCR4, Al18F-NOTA-pentixafor allows for the non-invasive localization of CXCR4-positive lesions. This imaging approach is being investigated as a potential alternative to invasive adrenal vein sampling (AVS) for subtyping primary aldosteronism. The aluminum-fluoride labeling strategy facilitates large-scale automated synthesis and centralized distribution, offering logistical advantages over Gallium-68 labeled variants.
