Alacepril is a long-acting, sulfhydryl-containing angiotensin-converting enzyme (ACE) inhibitor used primarily for the treatment of hypertension and congestive heart failure. It acts as a prodrug, being metabolized in vivo to captopril and desacetylalacepril. The antihypertensive effect is achieved by inhibiting ACE, which blocks the conversion of angiotensin I to angiotensin II—a potent vasoconstrictor—resulting in vasodilation and reduced blood pressure. Additionally, inhibition of ACE prevents the breakdown of bradykinin, further promoting vasodilation. Alacepril also reduces aldosterone secretion from the adrenal cortex, leading to increased sodium excretion and water outflow. It has been shown to have a longer-lasting antihypertensive effect compared to captopril and may also reduce peripheral sympathetic activity[1][2][3][4][5][7][8].
