Alagebrium is a small molecule drug developed as the first clinically tested "AGE-breaker," designed to break advanced glycation end-product (AGE) crosslinks in proteins. AGEs are implicated in aging and several degenerative diseases by causing protein crosslinking that leads to tissue stiffening. Alagebrium was intended to reverse vascular and myocardial stiffening associated with hypertension, heart failure (especially diastolic dysfunction), diabetes-related complications such as nephropathy and atherosclerosis, and other age-related conditions. Its mechanism involves cleaving AGE-induced protein crosslinks—primarily those formed by glycation—thereby improving tissue elasticity and function[3][5][9]. Preclinical studies showed benefits on vascular stiffness and cardiac function; however, clinical trials did not lead to regulatory approval or widespread clinical use[5]. The drug was originally developed by Alteon (later Synvista Therapeutics), but development was discontinued after phase II trials due to lack of efficacy or financial constraints[3][5].
