Alalevonadifloxacin (WCK 2349) is an oral prodrug of levonadifloxacin, a novel benzoquinolizine subclass fluoroquinolone antibiotic developed to address multidrug-resistant Gram-positive infections, particularly methicillin-resistant Staphylococcus aureus (MRSA). It is the L-alanine ester mesylate salt form of levonadifloxacin and achieves high oral bioavailability (~90%). Upon administration, it is rapidly converted in vivo to its active form. The drug acts by inhibiting bacterial DNA gyrase and topoisomerase IV—enzymes essential for DNA replication—leading to bactericidal activity. Alalevonadifloxacin has been approved in India for the treatment of acute bacterial skin and skin structure infections (ABSSSI), including diabetic foot infections with concurrent bacteremia. It has also received Qualified Infectious Disease Product (QIDP) designation from the US FDA. The drug demonstrates potent activity against MRSA, quinolone-resistant S. aureus, vancomycin-intermediate and -resistant strains (VISA/VRSA), as well as other difficult-to-treat Gram-positive pathogens[1][2][3][4][5][6][8].
