ALB-127158 is a potent and selective small molecule antagonist of the melanin-concentrating hormone receptor 1 (MCHR1). It was developed as an anti-obesity agent, aiming to reduce food intake and body weight by inhibiting the central effects of MCH, a neuropeptide involved in energy homeostasis. Preclinical studies demonstrated significant weight loss in rodents due to reduced food intake. In phase 1 clinical trials involving healthy lean and overweight/obese male subjects, the compound was safe and well tolerated but showed only modest effects on hunger and desire to eat at higher doses than predicted from preclinical models. Limited brain exposure in humans compared to animal models led to insufficient efficacy for obesity treatment, resulting in discontinuation of its development for this indication. There is some evidence suggesting that peripherally selective MCH1 antagonists like ALB-127158 may have potential utility in inflammatory bowel disease (IBD), though no clinical development has been reported for this indication[2][3][5].
