Albumin + saline refers to the combined use of human serum albumin (a plasma protein derived from human blood) and isotonic saline (sodium chloride solution) for intravenous administration. This combination is primarily used in clinical settings for fluid resuscitation, particularly in critically ill patients with hypovolemia, burns, or shock. Albumin acts as a colloid plasma expander by increasing oncotic pressure and drawing fluid into the intravascular space, while saline serves as a crystalloid volume expander that increases extracellular fluid volume. The combination may be chosen when both rapid plasma expansion and maintenance of oncotic pressure are desired. The SAFE trial demonstrated no significant difference in mortality between 4% albumin and normal saline for fluid resuscitation in ICU patients[5]. Albumin's mechanism includes maintaining colloidal osmotic pressure, stabilizing circulating blood volume, transporting various substances in the blood, providing antioxidant effects, and supporting capillary membrane integrity[1][4]. Saline provides isotonic volume expansion but does not contribute to oncotic pressure.
