ALDHbr umbilical cord blood cells are a specialized population of hematopoietic stem and progenitor cells (HSPCs) isolated from umbilical cord blood based on high expression of the enzyme aldehyde dehydrogenase (ALDH). ALDH is a marker for primitive, highly proliferative stem cells that are resistant to oxidative stress. These cells are used as an adjunctive cell therapy to augment standard unrelated umbilical cord blood transplantation (UCBT). The primary goal of supplementing a standard transplant with ALDHbr cells is to accelerate hematopoietic recovery (engraftment) and reduce the duration of severe cytopenia, which is a major cause of morbidity and mortality in cord blood transplant recipients. The cells are typically isolated using a fluorescent substrate for ALDH (such as Aldefluor) followed by fluorescence-activated cell sorting (FACS). In clinical trials, these cells have been evaluated both as freshly sorted populations and after cytokine-mediated priming or expansion in culture.
