Aleglitazar is a synthetic small molecule that acts as a dual agonist of peroxisome proliferator–activated receptor alpha (PPARα) and gamma (PPARγ). It was developed by Hoffmann–La Roche for the treatment of type 2 diabetes mellitus, aiming to improve both glycemic control and lipid profiles. By activating both PPARα and PPARγ, aleglitazar decreases plasma glucose, LDL cholesterol, triglycerides, and increases HDL cholesterol. Despite promising results in early clinical trials showing synergistic effects on lipid and glucose levels with limited toxicity, phase III studies were terminated due to lack of efficacy in reducing cardiovascular events and increased safety concerns[1][2][3][10].
