Alemtuzumab + rituximab is a combination of two monoclonal antibodies used primarily in the treatment of hematologic malignancies and severe immune-mediated diseases. Alemtuzumab is a humanized IgG1 monoclonal antibody targeting CD52, expressed on T and B lymphocytes, leading to profound lymphocyte depletion via mechanisms such as antibody-dependent cell-mediated cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and induction of apoptosis. Rituximab is a chimeric IgG1 monoclonal antibody directed against CD20 on B cells, mediating its effects through ADCC, CDC, and apoptosis induction. The rationale for combining these agents lies in their complementary activity: alemtuzumab depletes both T and B cells (with greater effect in blood/marrow), while rituximab targets B cells (with primary effect on nodal disease). This combination has been studied for refractory chronic lymphocytic leukemia (CLL), steroid-refractory autoimmune cytopenias such as autoimmune hemolytic anemia (AIHA) or immune thrombocytopenic purpura (ITP), as well as steroid-refractory chronic graft-versus-host disease (cGvHD). Clinical studies have shown that the regimen can be safe, feasible, with notable clinical activity especially where single-agent therapies are insufficient[1][2][3][4][5].
