Aletekitug is a humanized IgG1κ monoclonal antibody that specifically binds and neutralizes mature soluble interleukin-18 (IL-18), a pro-inflammatory cytokine in the IL-1 family that drives interferon-gamma production and Th1-skewed immune responses.[1][3][5] Developed originally by GlaxoSmithKline, the antibody shows high-affinity binding to IL-18 and prevents its interaction with the IL-18 receptor, thereby dampening downstream inflammatory cascades.[1][5] Aletekitug is being clinically developed primarily for moderate-to-severe atopic dermatitis, with prior and exploratory development in Crohn’s disease, inflammatory bowel disease (including IL-18–driven very-early-onset IBD), delayed graft function after kidney transplantation, type 2 diabetes mellitus in obese patients, and Behçet’s disease.[1] It has been administered intravenously in earlier studies and more recently subcutaneously, and has demonstrated clear target engagement with variable efficacy across indications, including a notable compassionate-use response in an IL-18opathy-driven VEOIBD case.[1]
