ALFQ (Army Liposome Formulation with QS-21) is a potent liposome-based vaccine adjuvant developed by the Walter Reed Army Institute of Research (WRAIR) and the U.S. Army Medical Research and Development Command. It is a next-generation formulation of the Army Liposome Formulation (ALF) that incorporates two powerful immunostimulants: synthetic monophosphoryl lipid A (specifically 3D-PHAD), which acts as a Toll-like receptor 4 (TLR4) agonist, and the saponin QS-21. These components are embedded within a phospholipid bilayer typically composed of DMPC (1,2-dimyristoyl-sn-glycero-3-phosphocholine) and DMPG (1,2-dimyristoyl-sn-glycero-3-phospho-rac-glycerol), along with cholesterol. ALFQ is designed to elicit robust and balanced immune responses, including high antibody titers and strong Th1-biased cellular immunity. It has been evaluated in clinical trials for various infectious diseases, including HIV-1, malaria, and COVID-19, often as part of candidate vaccines to optimize immunogenicity and safety.
