ALG-135041 is a potent and selective small molecule agonist of the thyroid hormone receptor beta (THR-beta), currently under development by Aligos Therapeutics for the treatment of metabolic dysfunction-associated steatohepatitis (MASH), formerly known as non-alcoholic steatohepatitis (NASH). By specifically targeting the beta isoform of the thyroid hormone receptor, which is predominantly expressed in the liver, ALG-135041 aims to improve hepatic lipid metabolism, reduce liver fat, and lower circulating cholesterol levels. This selective approach is designed to minimize the off-target effects associated with THR-alpha activation, such as cardiac toxicity (tachycardia) and bone mineral density loss. Preclinical studies have demonstrated that ALG-135041 significantly reduces liver steatosis and improves lipid profiles, supporting its clinical evaluation for safety and efficacy in patients with chronic liver diseases.
