Alirinetide is a synthetic six-amino-acid peptide analog of motoneuronotrophic factor 1 (MNTF1), developed by Genervon Biopharmaceuticals for the treatment of neurodegenerative diseases. It can cross the blood-brain barrier and is designed to induce an embryonic-like neuroprotective microenvironment, supporting neuron survival and self-correction of central nervous system pathophysiology[1][3][6]. Alirinetide acts through modulation of multiple signaling pathways, including insulin receptor, Notch receptors 1–4, patched 1, and smoothened frizzled class receptor. These interactions activate downstream cascades such as insulin signaling, Notch intracellular domain signaling, mitogen‐activated protein kinase pathways, and transcriptional networks involved in apoptosis inhibition, neurogenesis modulation, synaptic transmission regulation, glutamate clearance enhancement, anti-inflammatory responses and cytokine production[8]. The drug has completed phase II clinical trials for amyotrophic lateral sclerosis (ALS), Parkinson’s disease and stroke[2][3][4]. It has received orphan drug status for ALS in both the US and Europe[5].
