Overview

**Aliskiren + enalapril** is a combination of two antihypertensive drugs acting on the renin-angiotensin-aldosterone system (RAAS) at different sites. **Aliskiren** is a direct renin inhibitor that blocks the conversion of angiotensinogen to angiotensin I, leading to reduced formation of angiotensin II, a potent vasoconstrictor[6][9][3]. **Enalapril** is an angiotensin-converting enzyme (ACE) inhibitor prodrug that, once converted to enalaprilat, inhibits the conversion of angiotensin I to angiotensin II and decreases aldosterone secretion[1][2][4]. The combination provides dual blockade of the RAAS, potentially offering greater blood pressure reduction than either agent alone, but with safety concerns around kidney effects, hypotension, and hyperkalemia, especially in patients with diabetes or renal impairment[5][3]. This combination is not widely marketed under a unique brand name due to significant safety warnings and contraindications.
