Overview

**Aliskiren + ramipril + amlodipine** is a theoretical triple combination antihypertensive (blood pressure-lowering) medication that contains three active pharmaceutical ingredients: aliskiren (a direct renin inhibitor), ramipril (an angiotensin-converting enzyme inhibitor or ACE inhibitor), and amlodipine (a calcium channel blocker). Each drug targets a distinct pathway involved in blood pressure regulation: - **Aliskiren** blocks the activity of renin, reducing the production of angiotensin I and II, thereby causing blood vessels to relax and decrease blood pressure. - **Ramipril** inhibits the angiotensin-converting enzyme, which further prevents the formation of angiotensin II, leading to vasodilation and reduced aldosterone-mediated fluid retention. - **Amlodipine** inhibits L-type calcium channels in vascular smooth muscle, resulting in vasodilation and decreased peripheral resistance. This combination leverages complementary mechanisms to achieve more comprehensive blood pressure control in patients with hypertension not adequately managed by dual therapy. While dual combinations such as aliskiren + amlodipine or ramipril + amlodipine are approved and studied, there is currently no marketed or widely studied fixed combination product containing all three agents together[1][2][4]. This triple combination is primarily of theoretical or experimental interest, intended for advanced or resistant hypertension when dual therapy is insufficient.
