Overview

Alixorexton is a novel, investigational, once-daily, oral small molecule that acts as a selective agonist of the orexin 2 receptor (OX2R). Developed by Alkermes, alixorexton is being studied as a potential treatment for narcolepsy type 1 (NT1), narcolepsy type 2 (NT2), and idiopathic hypersomnia (IH). In phase 2 studies (Vibrance-1), alixorexton demonstrated statistically significant and clinically meaningful dose-dependent improvements in wakefulness (measured by Maintenance of Wakefulness Test), excessive daytime sleepiness (Epworth Sleepiness Scale), and cataplexy rates, as well as improvements in fatigue and cognition. Alixorexton was generally well tolerated in trials with no serious treatment-emergent adverse events. It is the first macrocyclic orexin receptor 2 agonist to reach late-stage clinical development, aiming to address symptoms resulting from orexin deficiency such as those characterizing narcolepsy with cataplexy. Its mechanism is direct stimulation of OX2R signaling pathways that regulate wakefulness and arousal.
