Overview

This is an alkalinized formulation of mepivacaine chlorhydrate (2%) and epinephrine, specifically diluted with 1.4% sodium bicarbonate. Developed and studied by Campus Bio-Medico University, it is intended for local infiltration anesthesia during ambulatory phlebectomy procedures for varicose veins. Mepivacaine is an amide-type local anesthetic that works by blocking voltage-gated sodium channels in nerve cell membranes, thereby preventing the initiation and conduction of pain signals. Epinephrine is included as a vasoconstrictor to prolong the duration of the anesthetic effect and minimize systemic absorption. The addition of sodium bicarbonate serves as an alkalinizing agent to raise the pH of the solution, which is intended to reduce the stinging pain associated with the infiltration of the typically acidic local anesthetic solution and potentially speed up the onset of action.
