Allium hookeri water extract (AHWE) is a botanical extract derived from the roots of *Allium hookeri*, a medicinal plant indigenous to Southeast Asia and widely cultivated in South Korea. The extract is characterized by its high content of sulfur-containing compounds, particularly cycloalliin. AHWE is primarily investigated for its potential therapeutic applications in metabolic diseases, including obesity, type 2 diabetes, and atherosclerosis. Its mechanism of action involves the modulation of lipid metabolism and inflammatory pathways. Specifically, it activates the ABCA1/LXRα/PPAR-γ signaling pathway to promote cholesterol efflux in macrophages, thereby inhibiting the formation of foam cells, which are critical in early-stage atherosclerosis. Additionally, AHWE exhibits anti-inflammatory properties by suppressing the expression of NF-κB, COX-2, and pro-inflammatory cytokines such as TNF-α and IL-6. Clinical research has included randomized, double-blind, placebo-controlled trials evaluating its efficacy in improving glycemic control in subjects with prediabetes.
