Allogeneic adipose-derived mesenchymal stromal cells expressing CXCR4 and IL10 is a genetically modified cell therapy developed by the Fundacion Instituto De Investigacion Sanitaria De Navarra (IdiSNA) and Navarrabiomed. It is specifically designed for the treatment of acute graft-versus-host disease (aGVHD) that is refractory to corticosteroids and ruxolitinib. The therapy utilizes mesenchymal stromal cells (MSCs) derived from healthy donor adipose tissue, which are then engineered to overexpress the C-X-C chemokine receptor type 4 (CXCR4) and the anti-inflammatory cytokine Interleukin-10 (IL-10). The CXCR4 modification aims to improve the homing and retention of the MSCs at sites of inflammation and tissue damage, where the ligand CXCL12 is upregulated. Once at the target site, the continuous production of IL-10 by the MSCs exerts a potent immunosuppressive effect, inhibiting the activation of donor T cells and promoting an anti-inflammatory environment to alleviate the symptoms of GVHD.
