Overview

Allogeneic bone marrow transplantation (BMT) is a cell-based therapy involving the infusion of hematopoietic stem cells obtained from the bone marrow of an HLA-compatible donor into a recipient. This procedure is primarily utilized for the treatment of high-risk hematologic malignancies, including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), chronic myeloid leukemia (CML), myelodysplastic syndromes (MDS), and myeloproliferative disorders. The therapeutic effect is twofold: first, the donor cells reconstitute the recipient's hematopoietic system following myeloablative or reduced-intensity conditioning; second, the donor-derived immune cells exert a graft-versus-leukemia (GVL) effect, identifying and eliminating residual malignant cells. The Medical College of Wisconsin served as the lead sponsor for the Phase III BMT CTN #0201 trial, which compared the efficacy and safety of bone marrow-derived cells against G-CSF-mobilized peripheral blood stem cells in the unrelated donor setting.
