Allogeneic CART-NKG2D cells are an investigational chimeric antigen receptor (CAR) T-cell therapy being developed by the Instituto de Investigación Hospital Universitario La Paz. This 'off-the-shelf' therapy utilizes T cells derived from healthy donors that are engineered to express a CAR incorporating the extracellular binding domain of the human NKG2D (Natural Killer Group 2 Member D) receptor. This design allows the T cells to recognize and eliminate malignant cells expressing any of the eight known NKG2D ligands (MICA, MICB, and ULBP1-6), which are frequently upregulated in various hematological malignancies and solid tumors but have limited expression in healthy tissues. The therapy is currently being evaluated in the Phase I REALL_CART clinical trial, specifically targeting children, adolescents, and young adults with relapsed or refractory T-cell acute lymphoblastic leukemia (T-ALL).
