Allogeneic CD45RA-depleted memory T-cell donor lymphocyte infusion (DLI) is an investigational adoptive cell therapy developed by University Hospital Tuebingen. It is designed for patients with hematological malignancies who have undergone haploidentical hematopoietic stem cell transplantation (HSCT). The therapy involves the selective depletion of CD45RA-positive (naïve) T cells from donor lymphocyte collections using the CliniMACS system, resulting in a product enriched for CD45RO-positive memory T cells. This approach is based on the rationale that naïve T cells are the primary mediators of graft-versus-host disease (GVHD), while memory T cells retain the ability to provide protective immunity against viral pathogens (such as CMV) and contribute to the graft-versus-leukemia effect. By infusing these memory T cells post-transplant, the therapy aims to accelerate immune reconstitution and prevent disease relapse without inducing severe GVHD.
