This experimental allogeneic adoptive cell therapy, developed by the National Cancer Institute (NCI), utilizes costimulated tumor-derived donor lymphocytes (TDL) for the treatment of B-cell malignancies in patients who have relapsed after allogeneic hematopoietic stem cell transplantation (AlloSCT). The therapeutic process involves resecting tumor tissue from the patient, which contains donor-derived lymphocytes that have naturally infiltrated the malignancy. These lymphocytes are then isolated and costimulated ex vivo using anti-CD3 and anti-CD28 antibody-coated magnetic beads. This costimulation process is designed to activate and expand the population of T-bet+FoxP3− type 1 effector donor T cells, enhancing their graft-versus-tumor (GVT) potency. By using lymphocytes already present in the tumor microenvironment, the therapy aims to provide high tumor specificity while minimizing the risk of graft-versus-host disease (GVHD) compared to standard donor lymphocyte infusions.
