Allogeneic gamma delta (γδ) T cell infusion is an investigator-initiated cellular immunotherapy being developed by Donghua Zhang for the treatment of high-risk acute myeloid leukemia (AML). This therapy involves the infusion of donor-derived γδ T cells into patients following allogeneic hematopoietic stem cell transplantation (allo-HSCT) to prevent disease relapse. γδ T cells are a unique subset of T lymphocytes that recognize stress-induced ligands and phosphoantigens on tumor cells in a major histocompatibility complex (MHC)-independent manner, primarily through the γδ T-cell receptor (TCR) and the NKG2D receptor. Because their activation does not depend on MHC-peptide presentation, allogeneic γδ T cells can provide a potent graft-versus-leukemia (GVL) effect with a significantly lower risk of inducing graft-versus-host disease (GVHD) compared to conventional αβ T cells.
