Allogeneic hematopoietic cell transplantation (HCT) is a therapeutic cell therapy procedure being evaluated by the Fred Hutchinson Cancer Center for the treatment of severe, treatment-refractory Crohn's disease. The procedure, studied in the Crohn's Allogeneic Transplant Study (CATS), involves replacing a patient's diseased immune system with healthy hematopoietic stem cells from a matched sibling or unrelated donor. This approach aims to eliminate the genetic risk factors and abnormal immune responses to intestinal bacteria that drive Crohn's disease. The treatment protocol includes a conditioning regimen of cyclophosphamide, fludarabine, and low-dose total body irradiation to deplete the host's immune system, followed by the infusion of donor cells. Post-transplant, patients receive immunosuppressive medications such as tacrolimus and mycophenolate mofetil to prevent graft-versus-host disease (GVHD) and ensure successful engraftment of the new immune system.
