Overview

Allogeneic hematopoietic stem cell transplant (allo-HSCT) is a cellular therapy involving the infusion of hematopoietic stem cells from a genetically non-identical donor to a recipient. This specific protocol, utilized by the Masonic Cancer Center at the University of Minnesota, employs a myeloablative conditioning regimen consisting of the alkylating agents busulfan and cyclophosphamide. Busulfan and cyclophosphamide serve to eradicate malignant cells and provide necessary immunosuppression to facilitate donor cell engraftment. Following conditioning, the patient receives an infusion of allogeneic hematopoietic stem cells sourced from related or unrelated donor bone marrow, peripheral blood, or umbilical cord blood. The primary therapeutic objective is to establish a new hematopoietic system and leverage the graft-versus-leukemia (GVL) effect, where donor-derived immune cells recognize and eliminate residual malignant cells in patients with high-risk hematological malignancies such as acute lymphoblastic leukemia (ALL), acute myeloid leukemia (AML), and myelodysplastic syndrome (MDS).
