Overview

Allogeneic hematopoietic stem cell transplantation (HSCT) developed by the University of Rochester is a cellular immunotherapy protocol investigated for the treatment of relapsed or refractory metastatic renal cell carcinoma (RCC). This specific approach utilizes a non-myeloablative (reduced-intensity) conditioning regimen, typically involving agents such as fludarabine, cyclophosphamide, and anti-thymocyte globulin, to suppress the recipient's immune system and allow for the engraftment of HLA-matched sibling donor stem cells. Unlike traditional high-dose transplants, this method does not aim to eradicate the patient's bone marrow but rather to establish a donor-derived immune system. The primary therapeutic mechanism is the graft-versus-tumor (GVT) effect, where donor-derived T-lymphocytes recognize and eliminate the patient's malignant cells. The protocol may also incorporate donor lymphocyte infusions (therapeutic allogeneic lymphocytes) to further stimulate the anti-tumor response.
