Overview

Allogeneic hematopoietic stem cell transplantation (AHSCT) is a cellular therapy being evaluated by M.D. Anderson Cancer Center as an early salvage treatment for patients with primary induction failure (PIF) acute myeloid leukemia (AML). The procedure involves the infusion of donor-derived hematopoietic progenitor cells (typically CD34+ cells from bone marrow or peripheral blood) into a recipient following a conditioning regimen. In the specific protocol studied at M.D. Anderson, AHSCT follows a salvage chemotherapy regimen known as DCIA (decitabine, clofarabine, idarubicin, and cytarabine). The therapeutic effect is achieved through the reconstitution of the recipient's hematopoietic and immune systems and the induction of a graft-versus-leukemia (GVL) effect, where donor-derived immune cells recognize and eliminate residual leukemic cells.
