Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a cellular therapy used to treat high-risk hematologic malignancies. This specific protocol, developed by He Huang at Zhejiang University and evaluated in the MIGHT trial, utilizes HLA-mismatched unrelated donors for patients who lack suitable matched donors. The procedure involves a conditioning regimen—including myeloablative or reduced-intensity doses of busulfan, fludarabine, cyclophosphamide, and semustine—to eradicate the patient's underlying disease and suppress the immune system to allow for donor cell engraftment. The therapeutic effect is derived from the reconstitution of a healthy hematopoietic system and the 'graft-versus-leukemia' (GVL) effect, where donor-derived immune cells recognize and eliminate residual malignant cells.
