Allogeneic lymphocytes, specifically donor-derived T-cells, are utilized as an adoptive immunotherapy for patients with metastatic melanoma. This therapeutic approach involves a non-myeloablative conditioning regimen followed by an HLA-matched allogeneic hematopoietic stem cell transplant and subsequent donor lymphocyte infusions (DLI). The primary mechanism of action is the induction of a graft-versus-tumor (GVT) effect, where the infused allogeneic T-cells recognize minor histocompatibility antigens or tumor-specific antigens on the melanoma cells, triggering an immune response that leads to tumor regression. This strategy is designed to provide a continuous immune-mediated anti-tumor effect that is not achievable with standard chemotherapy or autologous cell therapies.
