Allogeneic mesenchymal stem cells from adipose tissue (AD-MSCs) is an investigational cell-based biologic therapy being evaluated for the treatment of acute ischemic stroke. The product consists of donor-derived mesenchymal stem cells harvested from human adipose tissue, which are expanded and administered as a single intravenous infusion. The therapy's mechanism of action is multifaceted, focusing on the promotion of neurovascular unit repair and the enhancement of brain plasticity rather than targeting a single molecular pathway. It is characterized by low immunogenicity due to the lack of HLA class II expression on the MSCs. The development is led by the Instituto de Investigación Hospital Universitario La Paz (IdiPAZ) and FIBHULP in Spain, with clinical evaluation conducted through the AMASCIS trial program (NCT01678534, NCT04280003).
